Media Authorization Waiver

Thank you for volunteering at and/or donating to Homes with Hope. Please read and sign the Media Authorization, Waiver and Release Form below.

Authorization by Adult for Himself or Herself

I am eighteen (18) years of age or older and authorize Homes with Hope, Inc. and its representatives (“HwH”) to take photographs and videos, create digital images and likenesses of me and my property, and create audio using my voice. I also authorize HwH to: (i) copyright, use, post on websites or social media, or otherwise publish such photographs, videos, images and likenesses, and audio (“Material”)—-and any other photographs, videos, images, and likenesses and audio I may provide to HwH (“Other Material”)—-in print, audio, online, video or otherwise, at any time or times, and as many times (now or in the future), and for as long, as HwH chooses to copyright, use, post or otherwise publish them and (ii) authorize HwH’s funders and sponsors to copyright, use, post on websites or social media or otherwise publish the Material and the Other Material. I understand that neither HwH nor any of such funders or sponsors shall be obligated to compensate me in any way or at any time for any such copyright, use, posting, or publication. I agree that HwH and such funders and sponsors may use any of the Material and the Other Material with or without my name for any lawful purpose including, but not limited to, publicity, illustration, advertising, and website content.

Authorization by Parent or Guardian for Minor Child

I am eighteen (18) years of age or older and am the parent or legal guardian of a person who is under the age of eighteen (18) (“Child”), and make the statements contained in, and enter into, this agreement for and on behalf of Child. I authorize Homes with Hope, Inc. and its representatives (“HwH”) to take photographs and videos, create digital images and likenesses of Child and my property and Child’s property, and create audio using Child’s voice. I also authorize HwH to: (i) copyright, use, post on websites or social media or otherwise publish such photographs, videos, images and likenesses, and audio (“Material”)—-and any other photographs, videos, images, likenesses and audio I or Child may provide to HwH (“Other Material”)—-in print, audio, online,  video or otherwise, at any time or times, and as many times (now or in the future), and for as long, as HwH chooses to copyright, use, post or otherwise publish them and (ii) authorize HwH’s funders and sponsors to copyright, use, post on websites or social media or otherwise publish the Material and the Other Material. I understand that neither HwH nor any of such funders or sponsors shall be obligated to compensate me or Child in any way or at any time for any such copyright, use, posting, or publication. I agree that HwH and such funders and sponsors may use any of the Material and the Other Material of Child and my property and Child’s property with or without my name or Child’s name for any lawful purpose including, but not limited to, for publicity, illustration, advertising, and website content.

I waive any right to inspect or approve any materials or communications (including, but not limited to, any photographs, videos, audio, images, and likenesses) that use or incorporate any of the Material or the Other Material. I understand that if the Material or the Other Material, or any such materials or communications that use or incorporate them, could be viewed and downloaded on any computer or other electronic communication or storage device.

I release HwH and HwH’s employees, agents, invitees, and contractors from all claims and liabilities that I might assert at any time in connection with any of the matters described in this agreement. The terms and conditions of this paragraph shall survive the termination of this agreement for any reason.

The authorizations, waiver, and release described in this agreement shall remain in effect until I terminate this agreement in writing. No such termination shall be effective, however, with respect to the Material or the Other Material I authorized HwH to: (i) copyright, use, post on websites or social media or otherwise publish or (ii) authorize HwH’s funders and sponsors to copyright, use, and post on social websites or social media or otherwise publish, before the date of such termination.
